The diverse programs of Yolo Adult Day Health Center promote the well-being, dignity and self-esteem of an individual. Their goal is to maximize independence, improve management of chronic symptoms, prevent hospitalization and/or premature nursing home placement and provide support and relief to caregivers.
Yolo Adult Day Health Center Assists Individuals And Families Who:
- Have trouble with everyday tasks
- Need assistance in managing medication
- Need ongoing physical, occupational or speech therapy
- Are isolated, depressed, confused or suffering memory loss due to disease or a related condition
Specific Programming Includes:
Adult Day Health Care
A medical model program providing a team approach to managing complex chronic health conditions that impact independent living. Team members include participant's physician, nurses, social workers, rehabilitation specialists, recreation therapists, dietician and mental health professionals along with the participant and his/her caregivers. This dynamic interdisciplinary approach provides comprehensive support that includes all aspects of the individual’s life including home environment.
Nursing: Includes assistance with medications, monitoring blood pressure/blood sugar/general health and well-being, education, nursing treatments ordered by a physician and assistance in accessing other health care services. Nursing assistants provide personal care support such as toileting, feeding, hygiene, groom and even showering).
Social Work: Social work staff assist individuals and families with planning for long term care services, provide counseling regarding change and transitions, problem-solving financial and legal issues, accessing community resources and providing ongoing caregiver support groups.
Therapy: Physical, occupation and speech therapy programs are available to assist with maintaining, regaining and even improving a participant’s functional, cognitive, speech and swallow abilities.
Activity Programming: Center staff work closely with each person to develop programs that address individual preferences and skills. Attention is given to each person’s desires related to socialization, creativity and self-improvement. The focus is empowerment! In many ways, we like to think of ourselves as coaches and facilitators more than care providers in many ways.
Transportation: To and from the Center, including those in wheelchairs, may be arranged through the Center.
Specialized Dementia Services: The CASA Program, is a specialized program within a program, targeting the needs of individuals with a chronic neurocognitive disorder, especially Alzheimer’s disease, the most common form of dementia
Besides therapeutic activities, CASA provides needed respite to caregivers and offers caregiver support groups and other assistance for caregivers in coping with the multiple challenges of living with/caring for a loved one with dementia.
Adult Day Care
A social model program offering program participants a stimulating and safe environment with staff trained in dementia care and family caregiver support. Center also provides specialized staff training to meet the needs of individuals with moderate to severe dementia, who require additional assistance to benefit from the Center.
Caregiver Support Program
The Caregiver Support Program provides direct assistance to all community caregivers by offering care coordination, counseling, support groups, workshops and consultation for alternative living situations.
Senior Transportation Voucher Program (STVP)
STVP offers vouchers to any Yolo County Resident over the age of 60 who relies on YOLOBUS Special or Davis Community Transit for transportation to medical appointments, shopping, or other needs. This is made possible via a funds from Agency on Aging Area 4.
